1. Who we are
This policy applies to the IZORRA Thrive member-facing experience operated by Izorra.com LLC (collectively, “IZORRA Thrive,” “we,” “us,” or “our”). It covers this website, member accounts, program-interest and pilot applications, provider-network applications, contact requests, and first-party site analytics.
2. Information we collect
Depending on how you use the site, we may collect:
- program interest and pilot-application details, including first name, email address, ZIP code, individual or group intent, selected program, and a generated group identifier where applicable;
- provider-application details, including name, email address, optional phone number, organization, provider type, states of practice, optional license number, optional website, programs of interest, and application notes;
- contact details and the content of member, general, or provider-network inquiries;
- account information used by our authentication provider, such as email address, one-time sign-in codes, and membership approval status; and
- minimal first-party analytics data: event name, page route, action target, timestamp, and a random browser-tab identifier used for approximate visit counts.
IZORRA Thrive’s public forms do not request clinical histories, diagnoses, medications, laboratory results, biometrics, treatment records, or patient records. Please do not submit medical or health information through public forms or general contact messages.
3. How we use information
We use information to receive and review applications and inquiries; administer programs and accounts; understand member interests; communicate next steps; maintain security; prevent abuse; operate and improve the site; and comply with applicable law.
Submitting an application does not guarantee selection, enrollment, provider availability, clinical eligibility, treatment, or any outcome.
4. How we disclose information
We may disclose information to vendors that help us host, secure, authenticate, maintain, or support the platform; to professional advisers; when required by law; to protect rights, safety, or security; or in connection with a lawful corporate transaction. Providers receive information only as appropriate for an approved relationship or workflow and remain independently responsible for their own privacy obligations. We do not sell personal information or share it for cross-context behavioral advertising based on the current implementation.
5. Cookies and analytics
Authentication may use browser storage and related technologies needed to keep an account signed in. IZORRA Thrive also uses privacy-minimal, first-party analytics to understand key page visits and actions. The analytics system does not store names, emails, ZIP codes, form contents, IP addresses, full user-agent strings, advertising identifiers, or cross-site tracking identifiers, and the anonymous tab identifier is discarded when the browser tab closes. We do not currently use third-party advertising or session-replay tools.
6. Independent providers and clinical information
IZORRA Thrive manages the platform, program and member experience, participation, community, and provider-network coordination. Independent licensed providers are solely responsible for clinical evaluation, diagnosis, treatment decisions, prescriptions, monitoring, and follow-up. This policy does not replace a provider’s own privacy notice, consent process, or legal obligations.
IZORRA Thrive does not claim that the current platform or public intake process is HIPAA compliant.
7. Security and retention
We use administrative, technical, and organizational safeguards designed to protect information. No method of transmission or storage is completely secure. We retain information only as long as reasonably needed for the purposes described here, including operations, security, recordkeeping, and legal obligations.
8. Your choices and privacy rights
You may ask about, correct, or request deletion of personal information, or withdraw from future communications, subject to identity verification and applicable exceptions. Use the contact form to make a request.
Residents of California and other jurisdictions may have rights to know, access, correct, delete, or obtain a copy of personal information and to limit or opt out of certain uses. IZORRA Thrive does not discriminate for exercising applicable privacy rights. Where applicable law allows, you may use an authorized agent; we may ask for proof of authorization and verify your identity. If we decline a request, you may ask us to reconsider it through the contact form.
9. Children and minors
The service is intended only for adults aged 21 or older. It is not directed to anyone under 21, and we do not knowingly collect personal information from anyone under 21. If we learn that we have, we will delete it.
10. Changes and contact
We may update this policy as the platform or legal requirements change. The effective date will identify the current version.
