Apply to the IZORRA Provider Network.

We’re looking for licensed professionals who may participate in IZORRA member programs while maintaining independent clinical judgment and responsibility.

Participating providers remain solely responsible for clinical evaluation, diagnosis, treatment, prescriptions, monitoring, follow-up, licensure, and compliance with applicable law. IZORRA does not diagnose, prescribe, or treat.

Do not include patient information, medical records, or clinical notes.
